NAD+ (nicotinamide adenine dinucleotide) is a coenzyme central to how cells generate energy and repair DNA damage. From around the mid-thirties, its concentrations decline progressively across skin, blood, liver, muscle, and brain. A January 2026 clinical trial adds a significant data point on how effectively that decline can be addressed.

14 Days, Twice the NAD+

Healthy adults taking NMN (nicotinamide mononucleotide) or NR (nicotinamide riboside) at 1,000mg per day for 14 days saw their circulating NAD+ concentrations double compared to baseline. Both compounds serve as precursors in NAD+ biosynthesis, meaning the body converts them into NAD+ through separate but related enzymatic pathways.

What stands out is that NMN and NR produced comparable results under the same conditions. Neither compound has yet emerged as the clear winner in direct head-to-head comparisons.

Seven-Plus Human Trials on Safety

Between 2023 and 2024, more than seven human clinical trials evaluated NMN at doses of 900 to 1,250mg per day for periods ranging from 4 to 10 weeks. Across these trials, NMN was well tolerated with no significant adverse events. Mild gastrointestinal discomfort was reported by some participants but did not lead to discontinuation.

As of 2026, the regulatory landscape has become clearer in more markets, and NMN supplements are now more widely available. Retail pricing typically falls between $25 and $65 per month at therapeutic doses.

What Happens When NAD+ Declines

NAD+ is not simply a fuel molecule. It participates in several simultaneous roles inside the cell.

  • Mitochondrial energy production: NAD+ is essential for the electron transport chain, the process by which mitochondria generate ATP
  • DNA repair: The PARP enzyme family consumes NAD+ to repair strand breaks. When NAD+ is depleted, repair efficiency drops
  • Sirtuin activation: SIRT1 is a protein associated with longevity-related pathways. It requires NAD+ to function and is involved in regulating inflammation, stress responses, and metabolic homeostasis
  • Inflammation control: NAD+ depletion is linked to increased oxidative stress and heightened inflammatory signaling

These same pathways operate in skin cells, which is why researchers have been investigating whether restoring NAD+ levels might support skin health over time.

Skin Aging Evidence Is Still Limited

A critical distinction needs to be made. The fact that NAD+ participates in skin cell biology is not the same as saying NMN supplements demonstrably improve visible skin aging in humans.

Human clinical trials that directly measure skin outcomes with NMN are still limited. The majority of published research has focused on blood NAD+ concentrations, muscle endurance, fatigue scores, cardiovascular markers, and metabolic function. There is also no human trial data establishing that NMN extends lifespan.

Research on fish collagen oligopeptides has documented mitochondrial protection through the NAD+/SIRT1/PGC1-alpha pathway, illustrating that NAD+-related mechanisms do intersect with skin biology. But for NMN specifically, connecting restored NAD+ levels to visible skin improvement in humans requires clinical evidence that does not yet exist at scale.

NMN or NR: How to Choose

Both compounds raise NAD+ effectively based on current evidence, and this trial showed similar results from each. NR has historically been more affordable, while NMN has become more competitively priced as production has scaled in 2025 and 2026.

Given the absence of definitive head-to-head data, the practical choice between them comes down to price, availability, and individual tolerance.

Practical Information

  • Dose: Start at 250mg and assess tolerance before moving to 500 to 1,000mg/day
  • Timing: Morning with food. Some users report mild nausea on an empty stomach
  • Form: Capsule or enteric-coated tablet. Sublingual forms exist but most clinical data is based on oral capsules
  • Combinations: Resveratrol has been studied alongside NMN for SIRT1 synergy, but human trial evidence for the combination remains limited
  • Caution: If you are on blood pressure or diabetes medication, consult a physician before starting

What This Actually Means

Doubling circulating NAD+ in 14 days is a meaningful pharmacokinetic result. A growing body of safety data from 2023 to 2026 makes NMN a more credible option than it was a few years ago. What it is not, at this stage, is a proven solution for visible aging or a lifespan extension supplement.

If NMN interests you now, the most grounded framing is as long-term support for cellular energy metabolism, one element within a broader approach that includes sleep quality, resistance training, and blood sugar stability. Supplements rarely work in isolation, and NAD+ biology is no exception.
