A 12-week RCT of Ceylon cinnamon (Cinnamomum verum) 1.5 g/day simultaneously improving cycle regularity and insulin sensitivity in women aged 25~40 with irregular menses has been published. The clinical position of natural molecules for PCOS-leaning women has been re-validated.

Clinical Data

A double-blind RCT in 140 women aged 25~40 with irregular menses (cycle 35+ days or under 21 days) randomized 1:1 to Ceylon cinnamon 1.5 g/day or placebo. After 12 weeks, the primary endpoint was cycle regularity (28±4 days), secondary endpoints were bleeding volume + HOMA-IR + androgens.

The cinnamon arm showed:

  • Cycle regularity +42% (p<0.001)
  • Heavy bleeding -28%
  • Dysmenorrhea (VAS) -22%
  • HOMA-IR -18% (insulin resistance improvement)
  • Free androgen index -15%
  • Fasting glucose -8 mg/dL

Greatest effect in PCOS-diagnosed subgroup (46 women): ovulation recovery rate +52%.

Mechanism: Insulin + Androgen

Cinnamon’s main active compounds:

  • Cinnamaldehyde — most abundant, antidiabetic
  • Cinnamic acid — anti-inflammatory
  • Polyphenol Type A polymers (MHCP) — insulin mimetic
  • Coumarin — abundant in cassia (liver risk)

Mechanism of action:

1. Insulin Pathway:

  • Insulin receptor activation (insulin mimetic)
  • GLUT4 transporter expression increase
  • Fasting glucose -8~12% reduction
  • Insulin resistance -15~20%

2. Androgen Balance:

  • Insulin resistance → SHBG decrease → free androgen increase
  • Cinnamon normalizes insulin → SHBG recovery → androgen reduction
  • → improved hirsutism, acne, amenorrhea

3. Anti-inflammatory·Antioxidant:

  • TNF-α, IL-6 reduction
  • ROS neutralization
  • Reduced chronic inflammation → PCOS core

4. Uterine Smooth Muscle:

  • Smooth muscle relaxation
  • Bleeding regulation

Ceylon vs Cassia: Decisive Difference

90% of market cinnamon is cassia (Chinese cinnamon). Clinical use is Ceylon (Cinnamomum verum):

Ceylon (true cinnamon) — clinical standard

  • Cinnamomum verum
  • Coumarin 0.004% (safe)
  • 2,000mg/day long-term safe
  • Price $$$ (expensive)

Cassia (general) — food use only

  • Cinnamomum cassia/aromaticum
  • Coumarin 1%+ (liver toxicity risk)
  • 1,000mg/day chronic risks liver
  • Price $ (cheap)
  • Absolutely not recommended for supplements

Label must show “Cinnamomum verum” or “Ceylon cinnamon”.

PCOS and Natural Molecules

PCOS (polycystic ovary syndrome) has insulin resistance at its core. Natural molecule matrix:

  • Inositol myo+D-chiro 4g — first line (ovulation recovery)
  • Cinnamon 1.5g — insulin + androgen balance
  • NAC 600mg — antioxidant + insulin sensitivity
  • Vitamin D 4,000 IU — insulin + hormone
  • Spearmint tea — androgen reduction

Alternative or adjunct to prescription (metformin). Clinical data: matrix at 12 weeks → ovulation recovery 60~75% (similar to metformin 50~65%).

Natural Use

Ceylon cinnamon natural sources:

  • 1 teaspoon cinnamon powder = ~2.5g
  • Add to oatmeal, yogurt, coffee
  • Cinnamon tea (1 tsp + hot water)
  • 5× price difference from cassia, but safety decisive

Clinical Application

  • Standard dose: Ceylon cinnamon 1,500mg/day
  • Split dose: 500mg × 3 (with meals)
  • Timing: 30 min before meal (max blood sugar effect)
  • Onset: 4 weeks start, 12 weeks stable
  • Side effects: nearly none (using Ceylon)
  • Caution: pregnancy, anticoagulants (bleeding risk)
  • Interactions: diabetes meds (hypoglycemia risk), metformin synergy
  • Synergistic matrix: inositol + NAC + vitamin D + magnesium
